Admissions Procedure

Admissions

Admission to St. Joseph's is voluntary. St. Joseph's shall seek to provide comprehensive treatment services, to persons diagnosed with Alcohol Dependence, Alcohol Abuse, or Chemical Dependency without discrimination or regard for age, sex, sexual orientation, national origin, source of referral. To the best of our ability we work to provide services to individuals lacking insurance coverage. Referrals may be made by individual, family, clergy, physicians, employers, community agencies and courts.

Prerequisites

Applicant must:

1.  be "dry" and withdrawn from mood altering drugs for at least 7 days prior to admittance to St. Joseph's.

2.  be 18 years of age or older;

3.  be free of serious communicable disease;

4.  have no medical or surgical condition or mental disability requiring acute care in a hospital; and

5.  have no psychiatric condition that would interfere with addiction treatment.

Documentation

Properly completed "CONSENT FOR THE RELEASE OF INFORMATION" forms must be returned immediately upon receipt.

Basic Medical Examination  performed no more than 6 months prior to admission. (This includes all required lab work. Note: Must attach a copy of the CBC, ALT, Complete UA, RPR, and TB test result within last year.)

Psychosocial History and Evaluation  The assessment must be conducted and dated no more than 6 months prior to admission. If the applicant was treated by you within the past six (6) months you may submit your Clinical Summary and Discharge Summary but you must include an update. 

Assessment must support a diagnosis of Alcohol Dependence. Alcohol Abuse. or Chemical Dependency per the DSM-IV (or its successors) and include diagnosis on Axes I through V.

Copies of Discharge Summaries from previous treatment facilities only if they are within the last year. Please do no send any that are more than 12 months old.

Medication

If applicant has prescribed medication, the referral agent MUST assure the applicant a sufficient amount of the medication to last a minimum of 7 days.

Financial

St. Joseph's "Consent of Release of Financial Information" form must be completed and signed to allow St. Joseph's to verify funding. A copy of at least 2 of the following forms of identification must be attached to the form the applicant's.

A.  Birth Certificate,
B.  Social Security Card,
C.  Insurance Card,
D.  Medicaid Card/Public Assistance Card, or
E.  Picture I. D. or Driver's License

All requested documentation must be received and approved clinically, medically and financially by
St. Joseph's prior to confirming an admission date.

Financial Determination

Upon receipt and verification of funding information, a determination will be made for each applicant.

NOTE: Funding Verification from all 3rd party payors (medicaid, insurance, etc.) must be complete prior to an applicant's admission to St. Joseph's. The applicant must be willing to actively participate in the program. Treatment length of stay is dependent on client need.

Aftercare

During treatment, St. Joseph's staff work with each resident to determine plans and goals for continuing recovery. Applicants MUST be open to aftercare planning, which may include a stay of up to three (3) months in a Guest House, Halfway House or other alcohol and drug free residence upon successful completion of St. Joseph's inpatient program.

Placement Assistance

Individuals and agencies (referral source) who refer the applicant to St. Joseph's are encouraged to participate in the Aftercare Planning for applicants participating in our program. Applicants receiving an Administrative Discharge or a Discharge Against Therapeutic Advice are usually referred back to the referral source.

Family Cooperation

Each applicant MUST encourage their family and significant others to attend St. Joseph's Family Services. Family Services allows families the opportunity to become familiar with the disease of alcoholism/chemical dependency and with the continuing rehabilitation and recovery process of the resident upon discharge from St. Joseph's.

NOTE:
Children must be at least ten (10) years of age to attend Family Services.

Articles to Bring to Treatment

Applicant must be given St. Joseph's "Personal Item Listing for Applicants" and instructed to adhere to the guidelines related to Requirred Items, Appropriate Items and Inappropriate Items to bring to St. Joseph's.
